When I download a Word document from the web, and open it in Word, formatting looks off.

Word Online is just a slimmed-down version of Word, meant for basic edits, and the formatting options between the two are different. Why your document is behaving like that is a question only Microsoft can answer, but it's likely a result of those differences.
